San Fernando Health Centre - The 2,500 sq. ft. San Fernando Health Care Center provides health care services to the underserved populations in the downtown area as well as the community at large of San Antonio. The SFHC consists of a 2 exam rooms, a medical staff office, an intake room, a case manager’s office and a separate waiting area. The SFHC is easily accessible to public transportation. The services provided are: basic ambulatory, primary and preventive physical and mental health, diagnostic and treatment services. When extensive evaluation and treatments are beyond the scope of the SFHC, medical staff will refer patients to other facilities. The SFHC staff coordinates these activities and current collaborations include: University Health System, UTHSCSA - Nursing School, Methodist Healthcare Ministries, City of San Antonio - Community Initiatives & Health Departments and SAISD. These partners enable the SFHC to provide a wide range of social services. SFHC services are available by appointment and are provided to citizens as well as non-citizens regardless of their ability to pay.

Methodist Healthcare Ministries - is a faith-based, non-profit organization that was created to provide care through health-related programs and services that it owns and operates. One of the components is the Wesley Nurse Program. The Wesley Nurse Health Ministries® offers health and social services to communities through an RN who is based out of a church; it is now in 70 church sites in San Antonio and throughout South Texas. Health Education is also offered to clients and the general community by nurse educators. MHM also provides financial support to established organizations that are already effectively fulfilling the needs of the underserved in local communities through programs and services that they already operate.

City of San Antonio - Community Initiatives - The vision of the department is to provide leadership, develop collaborative strategies, maximize resources, and improve the quality of life within the San Antonio community. The goal is to coordinate and foster strategic partnerships with governmental and community-based agencies, like the SFHC, to leverage resources essential to strengthening families, developing human capital and sustaining a community safety net. The department has assigned a case manager to the SFHC to provide assistance with housing, utilities and special city programs throughout the year.
